I'm on my second Evoluent mouse, having worn out the first.
I thought they were pretty pricy but I was still interested and willing to spend that much if comfortable and not likely to irritate my RSI. While right-handed, I use my mouse in my left hand but they only have a wired left-handed version and I would want wireless. :(

They also only seem to have up to W7 drivers which made me wonder if they are still behind this product.

That's all I ever do too - so could probably get by on Windows default drivers.

Personally, I love the Evoluent VerticalMouse and didn't hesitate to purchase a new one when the original bit the dust.  I went through a variety of mice and trackballs and continued with wrist problems.  It really made a difference for me on the computer.  Too bad there isn't similar products for hedge trimmers and other similar tools.

Yeah, I'd probably get one if they had a left-handed wireless version. :( Maybe I'll write them and complain.

I did contact support and this is what I was told:

On our download page, we have the v4 software listed as being for Windows 8.  Windows 10 isn’t even officially released yet, so we don’t have any mention of it.  But our v4 software has been tested in Windows 10, and works fine in it.

I actually cannot figure out exactly what the other 2 buttons are actually currently mapped to.  All I know is that it is non-standard, at least on a PC.  I think it is really really dumb to sell a mouse that doesn't have standard button mapping out of the box - hence many complaints from people who can't get it to work at work because they can't download the extra software to remap those buttons sensibly.

I tried a wireless Logitech Trackball first, and while it was a huge improvement, painwise, its just that little bit too awkward for some tasks - and there's the risk of a new injury in the thumb-al area since you have to spin the trackball with your thumb.  So, that's been relegated to occasional use, such as with the laptop or when I'm watching Netflix on the Big Screen (which is away from my desk).

I won't know how the Evoluent might work for me until my new HD shows up here and I get it installed and configured with Windows.  I don't feel like trying to mess with it on this laptop.

It is my understanding, and Corrine's reply from tech support seems to verify that the mouse will work as a standard HID compliant mouse without installing any additional drivers or software. That is certainly how it should work, and how gaming and other unique mice with extra buttons work.

You say "other 2 buttons" - but a standard HID compliant mouse only has two buttons and would not have other buttons. Are you saying the standard left and right mouse buttons don't work as normal left and right mouse buttons?

The Evoluent VerticalMouse has six buttons.  The attached image shows the default settings, all of which can be reprogrammed with the Evoluent Mouse Manager.  In addition, using the secondary features of button 6, buttons 1-4 can be used in combination with button 6 as shown or programmed for different options.  There are so many selectable options, I had to make a second screen capture to scroll to the bottom.
